Botox Is Not a Filler: What Botulinum Toxin Actually Does to Your Nerves

Botox is often talked about as casually as a filler, but Botox does something fundamentally different. It doesn't fill a wrinkle.

Botox works by interfering with communication between nerves and muscles.

That is the reason wrinkles soften after treatment.

And if you're considering injecting it into your face, it's worth understanding exactly what that meansβ€”not simply hearing that Botox is "temporary," "FDA approved," or "commonly used."

First: What Is Botox?

Botox is a brand name for onabotulinumtoxinA, a pharmaceutical form of botulinum neurotoxin type A.

Botulinum neurotoxins originate from Clostridium botulinum, the bacterium associated with botulism.

That does not mean receiving Botox is the same thing as having botulism, nor does the injectable contain live C. botulinum bacteria. But it does contain botulinum toxin- widely recognized as one of the most potent biological toxins known to science. Capable of causing severe paralysis at extremely small amounts. It has even been developed as a biological warfare agent.Β 

The drug uses the neurotoxin's defining biological ability: interrupting nerve signaling.

That isn't a side effect.

That's how Botox works.

What Happens After Botox Is Injected?

Normally, your nerves send a message to your muscles telling them when to move. They do this by releasing a chemical called acetylcholine.

Botulinum toxin blocks that message.

It disrupts a protein called SNAP-25, which your nerves need in order to release acetylcholine. Without that signal, the muscle can’t contract normally.

The result is temporary muscle weakness or paralysis.

Because the muscle moves less, the skin above it creases less tooβ€”which is why expression lines can look smoother after Botox.

Botox Doesn't Actually Treat the Wrinkle

This distinction matters.Β Botox isn't filling a wrinkle, it is reducing the movement that creates or deepens it.

Your body has to recover from the interruption in nerve-to-muscle communication.


The Effects Don't Always Stay Exactly Where Botox Is Injected

One of the biggest misconceptions about Botox is that it simply β€œrelaxes one wrinkle.”

It isn't that simple.

Botulinum toxin products carry an FDA boxed warning because the effects of the toxin can, in some cases, spread beyond the injection site.

Reported effects include:

  • muscle weakness

  • drooping eyelids

  • double or blurred vision

  • difficulty speaking

  • difficulty swallowing

  • difficulty breathing

These reactions have been reported hours to weeks after injection.

Weakening one facial muscle can change how the muscles around it work.

Your facial muscles don't function independently. They pull against and balance one another every time you smile, squint, raise your eyebrows, or make an expression.

When Botox weakens one of those muscles, neighboring muscles may begin working differently to compensate.

That can sometimes create new movement patterns, asymmetry, or lines in surrounding areas.

Repeated Botox injections can also cause the treated muscle to become smaller over time. Research has documented muscle atrophy after repeated botulinum toxin exposureβ€”which makes sense, because a muscle that is repeatedly prevented from contracting normally is being used less.

This does challenge the idea that Botox simply β€œwears off and everything goes right back to normal” without affecting the muscle itself.

But underneath the skin, repeated cycles of weakening the same facial muscles can change their size, strength, and the way neighboring muscles compensate.

Research suggests botulinum toxin's activity may not always remain confined exactly where it is injected. Experimental studies have demonstrated transport along nerve pathways and toxin activity at sites away from the original injection, while the effects on nerve signaling can persist for months as the affected nerve endings recover.

FDA Approved Does Not Mean Risk-Free

This is where discussions about Botox often become oversimplified.

People hear:

"It's FDA approved."

And mentally translate that into:

"It's safe."

Those statements aren't equivalent.

FDA approval means a drug has been evaluated for particular indications and that regulators determined its benefits outweigh its known risks when used according to those approved conditions.

FDA-approved medications can still:

  • cause side effects

  • have contraindications

  • interact with other medications

  • cause serious adverse reactions

  • carry boxed warnings

Botulinum toxin products are a perfect example.

They can have legitimate medical benefits.

They can also produce a desired cosmetic effect.

And they still have risks.

All of those statements can be true simultaneously.

So, Is Botox "Safe If It's Done Correctly"?

That sentence is too simplistic.

No practitioner can guarantee that a patient will experience no adverse reaction.

A more scientifically accurate statement is:

Botox has a known risk profile. Some risks are relatively common and temporary, while others are uncommon but potentially serious. Technique and dose can influence risk, but they cannot make the procedure risk-free.

That is very different from simply calling Botox "safe."

Botox vs. Dermal Filler

Botox and filler are sometimes lumped together, but they work completely differently.

Botox:
Interferes with nerve signaling to reduce muscle contraction.

Dermal filler:
Adds material beneath or within tissue to restore volume, alter contours, or fill certain lines.

A filler does not intentionally block acetylcholine release from nerves.

But that doesn't make filler automatically "safer."

Dermal fillers have their own potential complications, including infection, nodules, inflammatory reactions and, rarely, accidental injection into a blood vessel. Vascular occlusion can cause tissue injury, and certain facial injections have even been associated with vision loss.

We deserve more than the statement, "Don't worryβ€”it's safe if it's done correctly."

We deserve to know what is being injected.

What I Recommend Instead of Botox

If you want to soften the look of expression lines without Botox, I prefer a gentler approach: Frownies-style facial patches, gua sha or facial massage, being mindful of repetitive expressions and facial tension, daily sun protection from large brimmed hats, and keeping the skin well hydrated with barrier-supportive skincare. For mature skin that is not acne-prone, I also recommend waterless cleansing with an oil cleanser only, rather than repeatedly stripping the skin with traditional face washing. The goal is to support the skin barrier, reduce unnecessary dryness and creasing, and encourage a more natural approach to aging without interfering with nerve-to-muscle communication.Β 

Skin Care Goes Beyond What You Put on Your Face

Supporting mature skin also includesΒ internal care and everyday habits, not just topical products. That means - avoiding fried foods & chips, plus keeping added sugar under 8g a day, choosing sourdough over white bread, eating antioxidant-rich foods like berries, drinking matcha, hibiscus tea, coconut water, and plenty of water; and limiting coffee to about one cup a day. Hydration can also be supported with water, lemon, and a small pinch of mineral salt in the morning. Yogurt sweetened with Manuka honey is a helpful addition as well.

Externally, keep things gentle: choose fragrance-free skincare, avoid ingredients that personally irritate or dry the skin, avoid all synthetic fragrances, use silk pillowcases, cleanse without over-stripping, and avoid exposing the face to unnecessary heat or steam; especially steam from cooking. None of these habits can stop aging, but together they can help support hydration, the skin barrier, and healthier-looking skin over time.
